CMA Healthcare provides in-home care services for Broward County residents enrolled in the Florida Medicaid Long-Term Care Program, also known as the Statewide Medicaid Managed Care Long-Term Care (SMMC LTC) Program. Our team supports individuals with respectful, reliable care to help them stay safe at home and in their communities.
What is the Medicaid Long-Term Care Program
The SMMC LTC Program is a Florida Medicaid program that helps eligible individuals receive long-term care services in the community instead of in a facility. Services are provided through managed care plans and are based on the individual’s needs and authorized care plan.
Note: Services and hours depend on eligibility and plan authorization.
Who CMA Healthcare serves
CMA Healthcare serves individuals in Broward County who are enrolled in the Florida Medicaid Long-Term Care Program. We support aged adults, adults with disabilities, and individuals with developmental disabilities based on the care plan and goals.
Services CMA may provide through LTC
Services are based on the plan’s authorization and the individual’s care plan. CMA Healthcare may provide:
Personal care assistance with Activities of Daily Living
Companion care and supportive supervision
Respite care for family caregivers
Homemaker services such as light housekeeping and meal preparation
Nursing services (LPN or RN) when authorized
Private duty or live-in care options when appropriate and authorized
For case managers and care coordinators
CMA Healthcare works closely with case managers, waiver support coordinators, and case management agencies to support safe, consistent services aligned with the care plan.
